Addiction to Suffering: The Clinical Challenges of Masochism
Nonsexual masochism (i. e. masochistic personality traits or “moral masochism”) refers to an enduring pattern of self-sabotaging behavior wherein the individual is drawn to situations which contribute to their own suffering. Although never considered a formal diagnosis, masochistic phenomena are alive and well in the treatment room. This presentation seeks to help clinicians make sense of the masochistic dynamics which are often present in the most challenging and confounding cases. It provides specific interventions for helping patients break free from this vicious cycle.
Learning Objectives
- Define the origins and evolution of the concept of masochism.
- Identify the criteria of self-defeating personality disorder as it appeared in the appendix of DSM-III-R.
- Identify common clinical presentations through an overview of Millon's subtypes.
- Discuss the intense countertransference reactions often elicited by masochistic patients.
- Identify 2-3 specific interventions to help clinicians tackle this difficult problem.
