Screening for Weight Suppression
The impact of both weight stigma and the limitations of gendered calculations within eating disorder treatment centers have been documented in concurrent research and patient accounts. Traditional methods for determining the need for weight restoration often exclusively rely on equations that include Ideal Body Weight via HAMWI or 50th percentile of CDC BMI-for-age growth curve data. Research has shown that upwards of 94% of people with eating disorders are in bodies with "normal" and higher BMI and that gender diverse people are up to 16x more likely to develop eating disorders. This research paired with diverse patient reports highlights the need for more inclusive and accurate methods to determine weight suppression.
This presentation will introduce a novel and inclusive method to assess weight suppression in both adults and adolescents. Weight suppression is a concept that has been discussed in eating disorder literature for years but no universally accepted tool has been systemically implemented thus far. In this talk, we discuss the development process, the implementation in a higher level of care, review preliminary research that examined practitioner acceptability and inclusivity effectiveness for over 3,000 patients, and debut the finalized assessment tool to the ED community.
This novel weight suppression assessment was designed to be utilized with all patients regardless of body size, eating disorder diagnosis, gender identity, and access to health care. This tool collects information including weight history, vitals, lab values, changes in endocrine function not explained by another medical intervention, reported physical symptoms related to GI and cognitive function, and reported eating disorder behaviors. This assessment does not rely on criteria that require cost prohibitive test metrics (DEXA scan) or familial histories that some may not have access to obtain. The identified metrics are then organized into weighted categories (Primary Objective, Secondary Objective, and Subjective Markers) to determine if a patient is weight suppressed, possible weight suppressed, or likely not weight suppressed. Based on weight suppression status, dietitians initiate either active weight restoration or nutrition restoration as an initial treatment plan. To ensure individualized care, weight suppression status is reassessed throughout treatment based on various factors.
